Should Patients Have Easy Access to Doctors’ Notes?

“You’re talking to your doctor, who is scribbling notes on a clipboard or typing on a keyboard. Maybe the note is something that seems a bit blunt, like “obese woman.” Or maybe it’s something you might misinterpret, like “SOB” (which, unless you have a particularly bad relationship with your physician, means “shortness of breath”).”
