New pilot provides ‘near real-time’ remote monitoring

“MedApps, Inc., a provider of mHealth technology, is launching a new remote monitoring pilot aimed at chronic disease management for patients with Congestive Heart Failure.
MedApps will partner with Meridian Health, a comprehensive healthcare provider in New Jersey, to initiate the pilot focused on patients discharged from acute care settings with CHF. The focus of the program will be to monitor the participants on a daily, near real-time basis, to help improve patient outcomes and decrease re-admissions to the hospitals within 30 days.”
