When Healthcare IT Isn’t a HIT

Steve Hernandez, Risk & Insurance Online

“Spurred by $34 billion in financial incentives from the U.S. stimulus package, healthcare organizations are racing to adopt information-technology products and services. Healthcare IT, or HIT, has been pitched as a panacea for improving patient care, reducing medical errors and increasing cost-saving efficiencies, and after 2015, instead of offering incentives, the government will penalize healthcare providers that don’t use it in a meaningful way.

MEDWISE: An Innovative Public Health Information System Infrastructure

Sahin YG, Celikkan U. Journal of Medical Systems, 2010

In this paper, we present MedWise, a high level design of a medical information infrastructure, and its architecture. The proposed system offers a comprehensive, modular, robust and extensible infrastructure to be used in public health care systems. The system gathers reliable and evidence based health data, which it then classifies, interprets and stores into a particular database. It creates a healthcare ecosystem that aids the medical community by providing for less error prone diagnoses and treatment of diseases. This system will be standards-compliant; therefore it would be complementary to the existing healthcare and clinical information systems.

Test-retest reliability in a computer-based medical history

Slack WV et al, J Am Med Inform Assoc, 2010

The authors developed a computer-based medical history for patients to take in their homes via the internet. The history consists of 232 ‘primary’ questions asked of all patients, together with more than 6000 questions, explanations, and suggestions that are available for presentation as determined by a patient’s responses. The purpose of this research was to measure the test–retest reliability of the 215 primary questions that have preformatted, mutually exclusive responses of ‘Yes,’ ‘No,’ ‘Uncertain (Don’t know, Maybe),’ ‘Don’t understand,’ and ‘I’d rather not answer.’

Economic externalities of health information technology. A game theoretic model for electronic health record adoption

Woodside JM. Journal of Healthcare Information Management, 21(4)

A presidential executive order in 2004 called for widespread adoption of electronic health records within 10 years. Proponents have shown this will lead to safe, affordable and consumer-oriented healthcare. Current EHR adoption has not kept pace; some estimates suggest that EHR adoption will occur over a significantly longer period. Implementation costs and return on investment are listed, among other reasons, as the predominant factors limiting rapid adoption. A widespread EHR adoption plateau is expected, with entities being unable or unwilling to adopt EHRs.
