“The Direct Project, a NHIN effort to provide a simplied, scalable, transport method for exchanging PHI will now be tested by the Rhode Island Quality Institue and the Hennepin County Medical Center in Minnesota. However, as I explained to a physician recently who confused the Direct Project with the state HIE, there are limitations to the Direct Project. It is designed only as a secure mechanism to transport and push PHI between two parties (It could be three parties if there is a Health Information Service Provider or HISP).

“There’s a pretty fierce battle going on right now between all the various stakeholders interested in exchanging patient data. The stakeholders range from very large companies to government initiatives to startup companies. One of the major problems that I see is that it’s not completely clear which model of patient data exchange will win out. In fact, let’s not be surprised if a number of different options take hold.

“The goal of an accountable care organization is to improve the integration of care among providers. This integration could encompass a wide variety of provider types, such as physicians, nurse practitioners, hospitals and non-acute providers. Ultimately, this integration should result in care that is safer, more efficient, and of better service and higher quality.

“We’ve known for years that health information technology can improve health care. But until recently, the implementation rate among providers has been low, except for a few early adopters.
In the last two years, however, there has been a significant upward inflection in the adoption rate. For primary care providers, adoption of a basic EHR increased by half from 19.8 percent in 2008 to 29.6 percent in 2010.

“One of the big concerns about implementing an Electronic Health Record (EHR) into a medical practice is the fear that it will slow you down. In fact, after cost, usability (and its effect on productivity) is the second-biggest perceived barrier to EHR adoption.

“It is hard to believe that this March will be exactly one year since President Obama signed the Patient Protection and Affordable Care Act (PPACA) of 2010. One major provision of PPACA is that its enactment requires the creation of a non-profit institute, the Patient-Centered Outcomes Research Institute, whose main responsibility is to undertake comparative effectiveness research.

“A recent post in the Wall Street Journal’s Health Blog noted that a study found electronic medical records don’t improve outpatient quality. The authors of the Archives of Internal Medicine article, Electronic Health Records and Clinical Decision Support Systems, correctly points out that we should be skeptical and “doubt [the] argument that the use of EHRs is a “magic bullet” for health care quality improvement, as some advocates imply.”

“Mobile health applications experience a second hype phase after first enthusiasm in the early 2000s. In 2011 there are uncounted conferences which focus on mHealth and the GSMA conference in Barcelona this week offers a special track on the potentials of mHealth applications. Companies like Qualcomm, HP, and Orange are trying to position themselves for the time when the market will take off. “In 2007, the chain of 26 hospitals put the brakes on an implementation of a new electronic health record (EHR) system from Epic Systems after it had already spent about $500 million on the project. The organization, which adopted the IBM i-based MedSeriesIV healthcare information system (HIS) from Siemens in the mid 1990s, announced last year that it would put an additional $400 million into the Epic project, with expectations of a full-roll out by 2015.

“We are not at a point where we can effectively: total, safely protect, easily transport and access our health data. I read a great article by Margalit Gur-Arie entitled, “Trust your life records to an unnamed chain of software vendors” she uses an analogy of our current banking system to emphasize how far away we are from gaining the trust around our health records data. Great article and highly recommended. This got me thinking about how we manage our data today?

“First, a true confession: I have two kids … and absolutely no idea when their yearly check-ups are. Originally, they were the same months as their birthdays, January and November. Simple, right? Not so much, since you can’t schedule next year’s appointment at this year’s appointment.
