Smyth discusses the recent report from the Academy of Medical Sciences on research regulation. The expert group’s reliance on anonymity to protect participants in research was based on assumptions about key NHS patient data systems that may no longer be justified in an era of ubiquitous data generation and sharing. The report also pays insufficient attention to patient autonomy.

“In de maatschappelijke discussie over het EPD domineren privacy en beveiliging. Bij beveiliging staan drie kwaliteitsaspecten centraal: vertrouwelijkheid, integriteit en continuïteit. De discussie wordt nu volledig gedomineerd door het aspect vertrouwelijkheid. Uiteraard is het cruciaal dat de toegang, gegevensuitwisseling en -opslag zeer goed zijn beveiligd.

“If anyone is suggesting that ICD-10 will not create a productivity drop in the days after compliance, they must be practicing silent protest. Most experts agree that come the flip-switch October 1, 2013 compliance deadline even seasoned, well-trained coders will take longer with the ICD-10 than they did with ICD-9. The overarching question is whether or not coders will master ICD-10 and, in so doing, resume ICD-9-era productivity?

Objectives
To study the satisfaction of end-users of a computerized physician order entry (CPOE) system concerning ease of use and the effect on users’ workflow, efficiency, and medication safety and to seek users’ opinions regarding required improvements of the system. Usability evaluation had shown that this system, which was in use for almost a decade, contained a number of severe usability problems. So another objective of the study was to determine whether there was a direct relation between user satisfaction and the results of a usability evaluation of the system.

McSpadden C, Spiro S. The Consultant Pharmacist, 26(3)
Consultant and senior care pharmacists probably have already encountered some sort of health information technology (HIT) system in one or more of the facilities they service, whether it is an electronic medication administration record or some version of an electronic health record (EHR).

Domingo M et al, Revista Espanola De Cardiologia, 2011
INTRODUCTION AND OBJECTIVES:
Multidisciplinary strategies for the management of heart failure (HF) improve outcomes. We aimed to evaluate the effectiveness of noninvasive home telemonitoring in ambulatory patients with HF already included in a structured multidisciplinary HF program.
METHODS:
Prospective intervention study with before/after comparison design of an interactive telemedicine platform in HF patients, randomized 1:1 into two groups: A) Motiva System with educational videos, motivational messages, and questionnaires, and B) Motiva System + self monitoring of blood pressure, heart rate, and weight. Hospitalizations were compared over 12 months prior to and post study inclusion. Quality of life was evaluated using the generic EuroQoL visual analogue scale and the specific questionnaire Minnesota Living With Heart Failure Questionnaire.

Objective:
To assess the impact of a home telemonitoring technology on patients with chronic obstructive pulmonary disease in terms of care satisfaction, patient empowerment, improved quality of life, and utilization of hospital and home care.
Design:
A quasi-experimental retrospective and prospective design was developed with a matched control group to compare the effects of telemonitoring (the experimental group, n = 23) with the traditional homecare offering (the control group, n = 23).

Herzberg S et al, International Journal of Medical Informatics, 2011
PURPOSE:
Medical documentation is often incomplete. Missing information may impede or bias analysis of study data and can cause delays. In a single source information system, clinical routine documentation and electronic data capture (EDC) systems are connected in the hospital information system (HIS). In this setting, both clinical routine and research would benefit from a higher rate of complete documentation.

Domingo M et al, European Journal of Cardiovascular Nursing, 2011
BACKGROUND:
Telemedicine can be useful for managing heart failure (HF), but patient acceptance of telemedicine and its impact on patient behavior are unclear. AIMS: To assess a telemedicine program in a HF Unit.
METHODS AND RESULTS:
This sub-analysis of the CARME study assessed the use of an interactive telemedicine platform. This prospective intervention study had a before/after design with HF patients randomized 1:1 into two groups: A) Motiva system (educational videos, motivational messages, and questionnaires); and B) Motiva system+telemonitoring of blood pressure, heart rate and weight. Of 211 patients screened, 44 were excluded, 62 did not consent to participate and 8 withdrew consent prior to installation of the system.

We conducted a systematic review of the applications and technical features of digital interactive television (DITV) in the health and social care fields. The Web of Knowledge and IEEE Xplore databases were searched for articles published between January 2000 and March 2010 which related to DITV systems facilitating the communication of information to/from an individual’s home with either a health or social care application. Out of 1679 articles retrieved, 42 met the inclusion criteria and were selected for review. An additional 20 articles were obtained from online grey literature sources. Twenty-five DITV systems operating in health and social care were identified, including seven commercial systems.

Equal access to mental health services is necessary for healthy individuals and communities. However, due to geographical distances and other barriers, some clients cannot easily access mental health professionals. Technologies such as videoconferencing for clinical purposes (i.e., telemental health) may help to bridge these gaps to connect clients and clinicians at geographically diverse locations. However, despite its potential utility, telemental health has not been widely adopted in Canada. This study is an exploratory investigation into mental health professionals’ attitudes toward telemental health, factors that affect the frequency with which they use this technology, and their perceptions of individual characteristics that make clients more or less suitable candidates for telemental health.

Uganda suffers from a severe shortage of professional healthcare workers. Thus, programs aimed at prevention of disease are an important complement to the limited healthcare delivery system. We analyze two deployments of an SMS-based HIV/AIDS education system that uses a quiz format to assess people’s knowledge of the disease, including its causes and methods of prevention. The deployments were to two groups in Uganda, one a sample of mobile phone users who live in a town in northwest Uganda; the other, workers at three factories in central and southeastern Uganda.

“Healthcare providers are doing whatever it takes to deploy electronic health record systems and all the related technology they’ll need to qualify for a share of the $27 billion in U.S. federal stimulus funds set aside to encourage investment in health IT. Their top priorities this year all relate in some way to the government’s financial incentive program, including meeting regulatory requirements, managing digital patient data, improving care, reducing costs, and increasing efficiencies.

Objective
Little evidence exists on effective interventions to integrate HIV-care guidelines into practices within developing countries. This study tested the hypothesis that clinical summaries with computer-generated reminders could improve clinicians’ compliance with CD4 testing guidelines in the resource-limited setting of sub-Saharan Africa.
Design
A prospective comparative study of two randomly selected outpatient adult HIV clinics in western Kenya. Printed summaries with reminders for overdue CD4 tests were made available to clinicians in the intervention clinic but not in the control clinic.
