This paper presents a unified framework of EHR usability, called TURF, which is (1) a theory for describing, explaining, and predicting usability differences; (2) a method for defining, evaluating, and measuring usability objectively; (3) a process for designing built-in good usability; and (4) once fully developed, a potential principle for developing EHR usability guidelines and standards. TURF defines usability as how useful, usable, and satisfying a system is for the intended users to accomplish goals in the work domain by performing certain sequences of tasks. TURF provides a set of measures for each of the useful, usable, and satisfying dimensions of usability.

Although most nursing students are proficient in data entry and computer skills, they often do not comprehend how the information they enter becomes a vital component of interdisciplinary team communication. Furthermore, the electronic medical record becomes a repository for information that can be retrieved for the purpose of decision support. Developed by the Cleveland Clinic, the Deans’ Roundtable, and University Hospitals of Cleveland, the Student Nurse Portal provides a means of assisting the student to understand how data entered into the computer transforms into information and knowledge, resulting in the wisdom that enables healthcare workers to provide optimal patient care.

We surveyed low-income, ethnically diverse consumers regarding their attitudes towards providers’ use of electronic health information exchange (HIE) and consumer use of HIE through personal health records (PHRs). Amongst respondents (n = 214), 48% had an annual household income below $15,000 and 62% spoke a language other than English at home. A majority indicated that they supported providers’ use of HIE (61%). Support for providers’ use of HIE was independently associated with consumer willingness to permit health care providers other than their primary care doctor to view their electronic medical record information (odds ratio (OR) = 2.92, 95% confidence interval (CI) = 1.31–6.50) and beliefs that electronic health record use would improve quality of care (OR = 2.70, 95% CI = 1.18–6.18). Seventy-eight percent would potentially use PHRs.

The global effort in the standardization of electronic health records has driven the need for a model to allow medical practitioners to interact with the newly standardized medical information system by focusing on the actual medical concepts/processes rather than the underlying data representations. An archetype has been introduced as a model that represents functional health concepts or processes such as admission record, which enables capturing all information relevant to the processes transparently to the users. However, it is necessary to ensure that the archetypes capture accurately all information relevant to the archetype concepts. Therefore, a semantic backbone is required for each of the archetype.

The study and practice of dermatology care using interactive audio, visual, and data communications from a distance is called teledermatology. A teledermatology practice (TP) provides teleconsultation as well tele-education. Initially, dermatologists used videoconference. Convenience, cost-effectiveness and easy application of the practice made “store and forward” to emerge as a basic teledermatology tool. The advent of newer technologies like third generation (3G) and fourth generation (4G) mobile teledermatology (MT) and dermatologists’ interest to adopt tertiary TP to pool expert (second) opinion to address difficult-to-manage cases (DMCs) has resulted in a rapid change in TP. Online discussion groups (ODGs), author-based second opinion teledermatology (AST), or a combination of both are the types of tertiary TP.

This paper presents an ethnographically inspired interpretive case study of the Electronic Medical Record (EMR) system at Sankara Nethralaya hospital in India. It presents challenges related to the adoption of the system and methods and strategies that were utilized in order to overcome these challenges and help the system be adopted successfully. One of the more notable challenges at the hospital was a user base that included skeptical users, those lacking computing skills, and that had a history of rejecting designs.

The Primary Care Information Project is a New York City initiative aimed at improving population health through the improved delivery of preventive care. It has assisted with the adoption of a fully functional electronic health record (EHR) in over 300 primary care practices. Practices with EHRs automatically transmit summary data that can be used to track population health indicators for recommended preventive care. Early analysis, focusing on small practices with fewer than 10 providers serving Medicaid and uninsured populations, showed increases in the delivery of recommended services of 0.1–2.4% per month (p≤0.05).

Implementing electronic health record (EHR) systems in the next few years is on the agenda for many healthcare organizations. Before investing in an EHR system, however, decision makers need to identify and measure the benefits of such systems. We propose using a system dynamics (SD) approach to measuring the benefits of EHR systems. Using an SD approach, it is possible to map complex relationships among clinical processes in hospitals into a model by which one can dynamically measure the effect of any changes in the parameters over time.
