When developing a “smart healthcare” application that takes the participants beyond their experience of the conventional analogue of that application, a challenge is to gather the requirements for the application and to map those requirements onto a successful design. Some requirements will come from the conventional way of delivering the healthcare service. Some will come from the domains of the technologies that have been used to create the smart healthcare application. Some requirements, however, particularly for point-of-care applications, will emerge from the interplay of the actual clinical situation and the novelty of the smart healthcare application.

Participant recruitment is an important process in successful conduct of randomized controlled trials. To facilitate enrollment into a National Institutes of Health–sponsored clinical trial involving patients with chronic obstructive pulmonary disease (COPD), we developed and prospectively validated an automated electronic screening tool based on boolean free-text search of admission notes in electronic medical records. During a 2-week validation period, all patients admitted to prespecified general medical services were screened for eligibility by both the electronic screening tool and a COPD nurse. Group discussion was the gold standard for confirmation of true-positive results.

We conducted a randomized, controlled pilot trial to evaluate the feasibility of providing subspecialty care via telemedicine for patients with Parkinson’s disease residing in a remote community located approximately 130 miles from an academic movement disorders clinic. Study participants were randomized to receive telemedicine care with a movement disorder specialist at the University of Rochester or to receive their usual care. Participants in the telemedicine group received three telemedicine visits over six months. Feasibility, as measured by the completion of telemedicine visits, was the primary outcome measure.
