“People with chronic medical problems like Parkinson’s disease can have a hard time finding a specialist who can help them manage the disease. Some patients are turning to doctors hundreds of miles away to get the care they need. But they’re not driving to get to the doctor. They’re doing the medical version of telecommuting, despite the fact that many insurers won’t pay for it.

Parkinson’s disease represents a chronic movement disorder, which is generally proportionally to age. The status of Parkinson’s disease is traditionally classified through ordinal scale strategies, such as the Unified Parkinson’s Disease Rating Scale. However, the application of the ordinal scale strategy inherently requires highly specialized and limited medical resources for interpretation. An alternative strategy involves the implementation of an iPhone application that enables the device to serve as a functional wireless accelerometer system.

Dorsey ER et al, Movement Disorders, 2010
We conducted a randomized, controlled pilot trial to evaluate the feasibility of providing subspecialty care via telemedicine for patients with Parkinson’s disease residing in a remote community located approximately 130 miles from an academic movement disorders clinic. Study participants were randomized to receive telemedicine care with a movement disorder specialist at the University of Rochester or to receive their usual care. Participants in the telemedicine group received three telemedicine visits over six months. Feasibility, as measured by the completion of telemedicine visits, was the primary outcome measure.

“A unique and innovative telemedicine project is providing nursing home patients that have Parkinson’s disease access to neurologists at the University of Rochester Medical Center (URMC). The effort is a joint initiative between URMC and the Presbyterian Home for Central New York in New Hartford, a 250 bed nursing home near Utica and about 150 miles from Rochester.

“This technology might come in handy to those who don’t want to haul their sick relatives with Parkinson’s all the way to a clinician for visual observation of the progress of disease. To test whether telemedicine is an appropriate option for getting patients and physicians together, and whether the technology is good enough to observe resting tremor, bradykinesia, rigidity, postural instability, and other symptoms of Parkinson’s, doctors Kevin Biglan and Ray Dorsey from University of Rochester Medical Center enrolled a group of elderly Parkinson’s patients from the Presbyterian Home for Central New York into a pilot study that has yielded promising results.”

“More than half a million Americans suffer from Parkinson’s disease, a brain disorder that leads to tremor and difficulty with walking, movement, and coordination. Although not a normal part of aging, Parkinson’s occurs most often among people 60 and older, and the risk increases with age. Older adults now can visit the NIHSeniorHealth Web site to learn more about Parkinson’s disease at www.nihseniorhealth.gov/parkinsonsdisease/toc.html. The Web site from the National Institutes of Health (NIH) is geared toward older adults and features clear language.”
