A service model for improving healthcare delivery in rural developing communities

The infrastructure needed in developing countries, especially in rural areas, often makes providing state-of-the-art healthcare cost prohibitive. We describe a highly asynchronous service model for healthcare delivery that is inexpensive, at least compared to the usual implementation of telemedicine, and involves technical service, public health, training and political aspects.

Access to emerging technologies and telehealth intervention modality preferences in rural patients

The importance of telehealth strategies in addressing the health needs of rural residents has been well documented. A core problem in enacting telehealth strategies for patient education in rural settings, however, is a lasting perception that rural residents do not have access to, comfort with, or willingness to use varying emerging technologies. The current study was undertaken to simultaneously investigate access to technologies, comfort with technologies, and willingness to participate in technology-based interventions among high-need rural clinical populations. A sample of 199 patients was recruited at a Federally Qualified Health Center (FQHC) in the rural South using convenience sampling. Participants completed a battery of assessments addressing technology access, use, and comfort as well as likelihood of participating in intervention modalities.

Perception, satisfaction and utilization of the VALUE home telehealth service

We conducted a randomized controlled trial to evaluate the perception, satisfaction and utilization of a home telehealth service for frail elderly people living independently in their home communities. Control group subjects continued with their usual care and intervention group subjects were able to supplement their usual care with the use of a web portal. The web portal allowed videoconferencing and electronic messaging between home care nurses and clients, ordering health-related and home care services, access to health-related information and general access to the Internet. A total of 99 eligible people (59 female, 40 male) from one urban and one rural study site agreed to participate in the study. Eighty-four subjects were active participants for nine months.
