Simulation is a disruptive and transformational technology in medicine

Stephen C. Schimpff, KevinMD

“Incredible innovative and entrepreneurial skills have led to many new techniques and technologies in medicine. Some are both disruptive of the old way of doing things. Others are transformational. Over the next few weeks I will review some of those that I believe are among the most disruptive and transformational.

Evaluating electronic health records systems: a system dynamics simulation

Kasiri N et al, Simulation, 2011

Implementing electronic health record (EHR) systems in the next few years is on the agenda for many healthcare organizations. Before investing in an EHR system, however, decision makers need to identify and measure the benefits of such systems. We propose using a system dynamics (SD) approach to measuring the benefits of EHR systems. Using an SD approach, it is possible to map complex relationships among clinical processes in hospitals into a model by which one can dynamically measure the effect of any changes in the parameters over time.

Engineering the electronic health record for safety: a multi-level video-based approach to diagnosing and preventing technology-induced error arising from usability problems

Borycki EM et al, Patient Safety Informatics - Adverse Drug Events, Human Factors and IT Tools for Patient Medication Safety, 2011

Electronic health records (EHRs) promise to improve and streamline healthcare through electronic entry and retrieval of patient data. Furthermore, based on a number of studies showing their positive benefits, they promise to reduce medical error and make healthcare safer. However, a growing body of literature has clearly documented that if EHRS are not designed properly and with usability as an important goal in their design, rather than reducing error, EHR deployment has the potential to actually increase medical error. In this paper we describe our approach to engineering (and reengineering) EHRs in order to increase their beneficial potential while at the same time improving their safety.

Using second life virtual simulation environment for mock oral emergency medicine examination

Schwaab J et al, Academic Emergency Medicine, 18(5)

Objectives:
Oral examination is a method used to evaluate emergency medicine (EM) residents and is a requirement for board certification of emergency physicians. Second Life (SL) is a virtual three-dimensional (3-D) immersive learning environment that has been used for medical education. In this study we explore the use of SL virtual simulation technology to administer mock oral examinations to EM residents.

Methods:
This was a prospective observational study of EM residents who had previously completed mock oral examinations, participating in a similar mock oral examination case scenario conducted via SL. EM residents in this training program completed mock oral examinations in a traditional format, conducted face to face with a faculty examiner. All current residents were invited to participate in a similar case scenario conducted via SL for this study. The examinee managed the case while acting as the physician avatar and communicated via headset and microphone from a remote computer with a faculty examiner who acted as the patient avatar. Participants were surveyed regarding their experience with the traditional and virtual formats using a Likert scale.

An intelligent virtual human system for providing healthcare information and support

Rizzo AA et al, An intelligent virtual human system for providing healthcare information and support, 2011

Over the last 15 years, a virtual revolution has taken place in the use of Virtual Reality simulation technology for clinical purposes. Shifts in the social and scientific landscape have now set the stage for the next major movement in Clinical Virtual Reality with the “birth” of intelligent virtual humans. Seminal research and development has appeared in the creation of highly interactive, artificially intelligent and natural language capable virtual human agents that can engage real human users in a credible fashion. No longer at the level of a prop to add context or minimal faux interaction in a virtual world, virtual humans can be designed to perceive and act in a 3D virtual world, engage in spoken dialogues with real users and can be capable of exhibiting human-like emotional reactions.

A business case for HIT adoption: effects of “meaningful use” EHR financial incentives on clinic revenue

Behkami NA et al, MEDINFO 2010, 2010

The goal of this study is to describe a framework that allows decision makers to efficiently evaluate factors that affect Electronic Health Record (EHR) adoption and test suitable interventions; specifically financial incentives. The United States healthcare delivery system is experiencing a transformation to improve population health. There is strong agreement that “meaningful use” of Health Information Technology (HIT) is a major enabler in this effort.

Wireless Body Area Network (WBAN) Design Techniques and Performance Evaluation

Khan JY et al, Journal of Medical Systems, 2010

In recent years interest in the application of Wireless Body Area Network (WBAN) for patient monitoring applications has grown significantly. A WBAN can be used to develop patient monitoring systems which offer flexibility to medical staff and mobility to patients. Patients monitoring could involve a range of activities including data collection from various body sensors for storage and diagnosis, transmitting data to remote medical databases, and controlling medical appliances, etc. Also, WBANs could operate in an interconnected mode to enable remote patient monitoring using telehealth/e-health applications.

Exploring virtual worlds for scenario-based repeated team training of cardiopulmonary resuscitation in medical students

Creutzfeldt J et al, J Med Internet Res, 12(3)

BACKGROUND:
Contemporary learning technologies, such as massively multiplayer virtual worlds (MMVW), create new means for teaching and training. However, knowledge about the effectiveness of such training is incomplete, and there are no data regarding how students experience it. Cardiopulmonary resuscitation (CPR) is a field within medicine in high demand for new and effective training modalities.

Avatar II: The Hospital

Stephanie Simon, The Wall Street Journal

“The nurses have been told there’s a crisis. But they’re hardly prepared for the chaos that awaits.
Dozens of patients, stricken with a debilitating flu, crowd the emergency room. Some slump mutely in chairs. Others wander, moaning or calling out for blankets. Just as the nurses begin triage, part of the hospital goes dark: a blackout.

Simulating Surgery to Reduce Implant Complications

“A computer simulation breakthrough could mean fewer medical complications and better surgical outcomes for patients undergoing hip, knee or spinal implant surgery. Each year surgeons across Europe perform a staggering 900,000 hip, knee and spinal implant operations. Implant surgery is one of the most remarkable advances in medical science. Such operations restore increased mobility and a vastly improved quality of life to millions of Europeans.”

Virtual reality and computer technology improve stroke rehabilitation

“Israeli hospitals have recently started to use virtual reality therapy for stroke patients.
One commonly used program has the patient watch his virtual image on a screen. For example, tennis balls are virtually thrown at the patient from all directions and the patients’ actual hand motions are recorded on screen.”
