“To see inside the digestive system, doctors have to thread a scope up or down – potentially doing damage along the way – or get their patients to swallow a capsule that tumbles through, uncontrolled, snapping random pictures.

Background
Today there is much debate about why telemedicine has stalled. Teleradiology is the only widespread telemedicine application. Other telemedicine applications appear to be promising candidates for widespread use, but they remain in the early adoption stage. The objective of this debate paper is to achieve a better understanding of the adoption of telemedicine, to assist those trying to move applications from pilot stage to routine delivery.

“As technology advances and demand for high-quality, efficient healthcare increases, teleradiology continues to be a bustling field of medicine and a fertile territory for laws. Teleradiology is a branch of telemedicine that involves the electronic transmission of radiological patient images (i.e., x-rays, MRIs, CTs) from one location to another location for the purpose of interpretation and consultation by radiologists who are not physically present at the transmitting location.

“Hospitals never close, but that doesn’t mean someone from every specialty is always on call. A growing number of facilities aren’t scheduling radiologists for overnight and weekend shifts, and others no longer have them on staff. Instead, they rely on teleradiology companies to fulfill their imaging needs.

The purpose of this article is to describe a system we developed for importing images on compact discs (CDs) from external imaging departments into our clinical image viewing system, and to report on key metrics regarding veracity of information seen on the CDs. We recommend careful attention to the process of CD importation because of the error rate we have seen. We developed a system and process for importing images on CD into our EMR. The importation system scans the CD for digital imaging and communications in medicine (DICOM) images, and collects all patient information seen.

Background:
Recent advances in the treatment of acute ischemic stroke have made rapid acquisition, visualization, and interpretation of images a key factor for positive patient outcomes. We have developed a new teleradiology system based on a client-server architecture that enables rapid access to interactive advanced 2-D and 3-D visualization on a current generation smartphone device (Apple iPhone or iPod Touch, or an Android phone) without requiring patient image data to be stored on the device. Instead, a server loads and renders the patient images, then transmits a rendered frame to the remote device.
Objective:
Our objective was to determine if a new smartphone client-server teleradiology system is capable of providing accuracies and interpretation times sufficient for diagnosis of acute stroke.

Objective:
Considerable expectations have been placed on information and communication technology (ICT) in improving the processes and quality of healthcare. Our purpose was to find out which element is found positive in healthcare ICT implementation.
Materials and Methods:
An online questionnaire on e-Health implementation submitted to all Finnish public health service providers and a sample from the private sector included an open question about which the electronic working methods, systems, or applications have most positively influenced the fluency or quality of service processes.

“With the help of tele-radiology, patients can now avail services of a radiologist without being physically present in the same room with them. There is a great need for professionals who can analyze patient repots, such as MRIs, X-rays, or CT scans, in a short duration so that patients do not have to wait for days to get the results.

“With increasing pressure on radiology departments and a shortage of radiologists, just handling and reporting on the everyday flow of scans can seem an insurmountable task, and as we’ve seen in recent years, reporting backlogs can build with frightening rapidity, putting patients at risk and exposing the hospital Trust to the risk of penalties.

Teleradiology can be used to provide health care to rural populations, especially where there is a scarcity of resources, including on-site radiologists. We have established a network link between a commercial teleradiology provider in Bangalore, south India and the Ramakrishna Mission Hospital (RKMH), located over 3000 km away in the north east of India. Image files were transferred to Bangalore via an ADSL connection using secure file transfer protocol. In the 12-month period beginning in August 2007, a total of 962 studies was sent to Bangalore from the RKMH.

“The healthcare industry is facing a critical time; physicians and staff, already inundated with patient needs, are being asked to perform more tasks in a broader spectrum of medical categories. While it’s important to be knowledgeable in a variety of medical specialties, with emerging and evolving technology, it is impossible to be a jack-of-all-trades. Outpatient care facilities must find alternative measures to ensure the highest level of patient quality care is maintained in both patient diagnosis and treatment.
