Online doctors make house calls again

“The digital doctor will see you anytime you want.
And that’s one reason Devon Herrick, an analyst with the Dallas-based National Center for Policy Analysis, says more doctors need to embrace technology and go online to give patients more access to health care.
The study, released Thursday, found not only are some doctors answering questions via e-mail, but they are also treating patients over the Internet. But Internet doctoring faces a mountain of questions about quality, liability, regulatory control and security before it becomes more accepted, according to a Connecticut-based patient advocate and the executive director of the Fairfield County Medical Association.”

Discussion on eConsultations

“Th WSJ Blog had a post yesterday on a rurl doctor’s support of eRx, but the limitations put upon such Internet practices by state regulations.
The WSJ set-up a separate discussion/forum entitled “Visiting you Doc Online” which is getting quite a few comments.  Granted, the WSJ attracts a very limited, dare I say exclusive, slice of the US population but it is this slice that is online, engaged, has insurance and based on the forum comments is fed up with the existing system.”

Digital diagnosis: Consultations get tech-savvy to save patients time

“Mike Holland came home from a business trip a few weeks ago in so much pain he tried something new: He made a doctor’s appointment online. The next morning, he arrived and saw the doctor in minutes.
A week later, on the road, his new medicine made him sick. The Hollywood computer consultant logged onto the same physician consulting Web site and filled out a form detailing the problem. Hours later, he had e-mail advice from his internist, Dr. Rene Reyes.”

E-consult reimbursement not new concept for some

“The CMS is proposing allowing physicians to bill for follow-up inpatient consultations delivered via telehealth, a move that could spur health insurers to do the same.
In the proposed fee-schedule rule released this week, and up for public comment until Aug. 29, the CMS suggests three new inpatient codes to bill for these consultations. The idea is such visits would be strictly for follow-up inpatient visits, and not subsequent hospital care, according to the CMS. A final rule will be issued Nov. 1.”

The virtues of virtual visits

“Rush-Presbyterian Medical Center’s Virtual Integrated Practice (VIP) is more evidence that remote health care can improve health outcomes.
At Rush, a team has been refining the VIP model for the past four years. The VIP’s objective is to improve chronic disease management for older people by deploying an interdisciplinary team using communications technology.”

What we have in health care today is a failure to communicate

“Communication in medicine grows worse by the day. What should be a pillar of quality health care is instead a resounding failure.
Patients are rushed through office visits and often leave without having their questions answered. Labyrinthine barriers have to be overcome before speaking with a physician. Reaching a medical provider via the Internet is an impossibly daunting task. Doctors rarely talk to each other to coordinate treatment plans.”

Virtual office visits appeal to patients, physicians, insurers

“Sitting at her home computer in the evening after work, she spent 10 minutes on a secure Web site answering the same kind of questions Dienst would have asked her during an office visit.
The next morning, she not only had heard back from her doctor, she had the prescription antibiotic and cough medicine that he had called in to her nearby pharmacy.”

e-Visit Data

“Patient online e-visits, introduced at the start of this decade, continue to gain momentum as technologies improve, consumer demand increases, experience from prior pilot studies becomes more widespread and major health plans advance and adopt e-visit initiatives. Here’s a collection of some recent data on e-visits, compiled in MCOL’s March @How-TO newsletter.”

Take two aspirin and e-mail me in the morning

“Dr. Joseph Scherger flew to Minnesota from his home in California, checked into a hotel in Brooklyn Park, and took care of 12 of his patients. All by e-mail.
That is the future of medicine, he told hundreds of doctors from Park Nicollet Clinic last week: Caring for patients without face-to-face visits.”
