Patients to get Facebook-style Communicator

“From March, patients will be able to carry out email consultations with GPs and other clinicians using a Facebook-style tool called Communicator.

Gap between SCR concept and reality

GP seeks urgent response on SCR legality

“A GP who is seeking publication of the Department of Health’s legal advice on plans for the Summary Care Record (SCR) has told the Information Commissioner (IC) he needs an urgent response to his request.
Dr Paul Thornton, a GP in Warwickshire, lodged a Freedom of Information Act appeal with the Information Commissioner in May after the DH refused to release its legal advice on the legal status of the health information stored as part of the SCR project.”

“The work is a qualitative study of attitudes of a group of patients representative of low health literacy, “potentially stigmatising conditions,” or difficulties in accessing health care regarding the National Health Service’ Shared Care Record (SCR), which appears similar to our Continuity of Care Record. It currently has medications, allergies, and adverse reactions, and is scheduled to include a minimum data set. Also discussed with the participants was the HealthSpace, which is a “personal health organizer available via the Internet,” so my guess is, similar to what we call a personal health record.”

Consent to view explored for detailed records

SCR consent model changes

Implied consent set to be scrapped for SCR

“The implied consent model for the Summary Care Record (SCR) looks set to be scrapped in favour of a simpler consent model following a recommendation from Connecting for Health’s advisory group.
Implied consent looks likely to be replaced by a model based on ‘consent to view’, providing a simpler more intuitive way for patients to decide who accesses their record.”

Patients’ attitudes to the summary care record and HealthSpace: qualitative study

Objective To document the views of patients and the public towards the summary care record (SCR, a centrally stored medical record drawn from the general practice record) and HealthSpace (a personal health organiser accessible through the internet from which people can view their SCR), with a particular focus on those with low health literacy, potentially stigmatising conditions, or difficulties accessing health care.
Design 103 semistructured individual interviews and seven focus groups.
Setting Three early adopter primary care trusts in England where the SCR and HealthSpace are being piloted. All were in areas of relative socioeconomic deprivation.
Participants Individual participants were recruited from general practice surgeries, walk-in centres, out of hours centres, and accident and emergency departments. Participants in focus groups were recruited through voluntary sector organisations; they comprised advocates of vulnerable groups and advocates of people who speak limited English; people with HIV; users of mental health services; young adults; elderly people; and participants of a drug rehabilitation programme.
Methods Participants were asked if they had received information about the SCR and HealthSpace and about their views on shared electronic records in different circumstances.
Results Most people were not aware of the SCR or HealthSpace and did not recall receiving information about it. They saw both benefits and drawbacks to having an SCR and described a process of weighing the former against the latter when making their personal choice. Key factors influencing this choice included the nature of any illness (especially whether it was likely to lead to emergency care needs); past and present experience of healthcare and government surveillance; the person’s level of engagement and health literacy; and their trust and confidence in the primary healthcare team and the wider NHS. Overall, people with stigmatising illness were more positive about the SCR than people who claimed to speak for “vulnerable groups.” Misconceptions about the SCR were common, especially confusion about what data it contained and who would have access to it. Most people were not interested in recording their medical data or accessing their SCR via HealthSpace, but some saw the potential for this new technology to support self management and lay care for those with chronic illness.
Conclusion Despite an extensive information programme in early adopter sites, the public remains unclear about current policy on shared electronic records, though most people view these as a positive development. The “implied consent” model for creating and accessing a person’s SCR should be revisited, perhaps in favour of “consent to view” at the point of access.”

Six Dorset practices added to SCR pilots

“Dorset Primary Care Trust has announced staff at six more local GP practices will start to use the NHS Summary Care Record this week.
The six GP practices – Milton Abbas, Gillingham, Newland in Sherborne, Cranborne, Swanage and Wool, will begin work on switching to the SCR this week. The Dorset practices form part of the fifth wave of surgeries to switch on the system as part of the Connecting for Health project.”

Reflection needed on SCR report says director

“The clinical director for the Summary Care Record has called for a period of reflection in the wake of the publication of the independent evaluation of the SCR.

Summary Care Record Early Adopter Programme: An Independent Evaluation by University College London

“A UCL (University College London) research team, led by London GP Professor Trisha Greenhalgh, has published its independent evaluation of the first year of the Summary Care Record (SCR) programme. The team found that although the SCR offers real benefits for treating patients in emergency and unscheduled care settings, the ‘”complicated” technical system needs to be refined before being rolled out. Nevertheless, both NHS staff and patients were largely positive or amenable to the programme, with most people valuing the benefits of instant access to medical records over the small risk of data loss or breach.
The SCR programme is an initiative by the English Department of Health to place a summary of key medical details (medication, allergies, known adverse reactions) of every NHS patient on a central database, accessible by NHS staff via a secure virtual network. It is currently being introduced in a number of ‘Early Adopter’ sites across England, of which the UCL team studied four. The year-long evaluation covered areas including: usability, usage and functionality of the SCR; impact and benefits of the SCR; patient access to their own SCR; evaluation of the Public Information Programme and evaluation of the consent/dissent model.
The evaluation team conducted extensive fieldwork within the early adopter Primary Care Trusts (PCTs), immersing themselves in the reality of implementation and usage of the SCR on the ground. The views and experiences of GPs, nurses, patients and the public, practice managers and other clinical and administrative staff using the SCR were captured.”

Personal electronic health records: MySpace or HealthSpace?

“The NHS, through Connecting for Health, is introducing two types of online health record for everybody in England—the summary care record and HealthSpace. Evaluation to recommend cautious SCR roll-out

“The pace of implementation of the Summary Care Record (SCR) looks likely to be slower than originally planned after it has emerged that only two PCTs have so far got records available to view over the Spine.
The first evaluation of the SCR, due to be published next month, is also expected to recommend a cautious approach to rollout and CfH has now commissioned a further two-year evaluation. However, CfH has denied that the additional evaluation will affect the timetable for national roll-out.”

Pharmacists may get access to patient care records

Northern Ireland launches Emergency Care Record
